IF YOUR CHILD HAS A LIFE-THREATENING FOOD/MILK ALLERGY, PLEASE HAVE YOUR PHYSICIAN FILL OUT THE FORM AND RETURN IT TO THE FOOD SERVICE DEPARTMENT.
